We prioritize protecting your private information and personal data. This statement of privacy applies to www.evergreen-therapy.ca and Evergreen Communication Therapy Ltd. (together with “Evergreen”, “we” and “us”). This Privacy Policy document contains types of information that is collected and recorded by Evergreen and how we use it.

It should be noted our privacy policies below are split into two main topics. 1) Data as it relates to Health information covered by PHIPA, and 2) data collected as it pertains to the engagement with our website. These policies are updated regularly.

1) Healthcare Information Policy

At Evergreen, confidential personal health information is protected by the Personal Health Information Protection Act (PHIPA). The Act ensures that the personal health information of our clients is confidential and kept secure by governing its collection, use and disclosure. Evergreen safeguards personal health information against unauthorized use, disclosure, copying, modification, and other unauthorized use including theft and loss.

Personal health information is collected, disclosed, and used with informed consent, and solely when it is necessary to provide an effective clinical service. As outlined by The Act, any client may withdraw consent by providing notice to Evergreen.

Access to Records

As a client, you can access your personal health information by requesting in writing to the contact person below. We will endeavour to respond to a formal access within 30 days. If necessary, we will provide written notice of the reason for any time extension and the length of the extension.

In very limited circumstances we may deny or restrict your access to your health record. For example, if it is reasonable to believe the disclosure would result in a risk of serious harm to you or another person, or in cases where a provincial or federal act prohibits disclosure.

2) Website Privacy Policy

Collection of Personal Information

Any information that you voluntarily provide to us, including your name, email address, will be used for the sole purpose for which the information was provided to us.

When you submit information to this website via webform, we collect the data requested in the webform in order to track and respond to your submissions. We share this information with Squarespace, our hosting provider, so that they can provide website services to us. We also share this information with info@evergreen-therapy.ca through channels of Google Mail for storage and with Zapier for data porting.

Sharing Information with Third Parties

Evergreen Communication Therapy Ltd. does not disclose, sell, rent or lease any information that is collected to third parties.
